Systemic Disease Produced By Vzv Reactivation Without Rash
Ross et al. reported a case of fatal hepatic necrosis caused by VZV in a 64-year-old woman. She had undergone splenectomy 14 months before the fatal hepatitis. The postmortem diagnosis was based on characteristic pathological changes in the liver, the detection of herpesvirus virions in liver by electron microscopy, and serologic evidence of recent VZV infection. A report of fulminant fatal disseminated VZV infection without rash occurred in an 8-year girl undergoing chemotherapy for leukemia virus was present in blood, lungs, liver, kidneys, and bone marrow . Although the nervous system was spared in both patients, these cases confirm the existence of disseminated VZV infection of multiple organs in the absence of rash.

According to the CDC, its safe to be around babies and young children, pregnant women or anyone with a weakened immune system after you get the Zostavax vaccine. And remember: You cant get shingles unless youve already had chickenpox.

Diagnoses And Treatment Of Shingles Without Rash
If you note that you are suffering from symptoms typically associated with shingles but without any rash it is important to tell your doctor right away. While you may not develop a rash, patients will still feel the tingling, burning sensation associated with the virus moving from the nerves to the skin. This sensation will typically only be on one side of the body. If you have had the chickenpox virus you will need to talk with your doctor to confirm your diagnosis. Because there are no blisters to check, your doctor may perform a blood test to check for the shingles virus. This will also help them rule out other conditions that cause similar symptoms.
Treatments
Once it is determined that you are suffering a shingles outbreak, your doctor will determine what level of care is appropriate. Many patients can be treated with medications like Valacyclovir or Acyclovir and home remedies such as cool baths and cool/wet compresses. In severe cases patients may need to stay in the hospital for treatment.
When Should I Seek Emergency Care For A Shingles Rash
While most cases of shingles are mild, several potential complications may require emergent care:
- Facial rash. A shingles rash on the face is concerning because eye involvement can lead to blindness. This requires an urgent assessment from an ophthalmologist.
- Loss of hearing or facial movement. Rarely, shingles can lead to hearing loss or facial nerve paralysis.
- Disseminated herpes zoster. This widespread shingles rash can also affect your organs. It requires hospitalization with intravenous antiviral treatment.
- Fever. Shingles in addition to fever can be concerning. It may mean you have a superimposed bacterial infection, which could require antibiotics and close observation.
- Confusion or seizures. Shingles in addition to confusion or seizures could indicate brain inflammation, which requires hospitalization for IV antiviral treatment and close monitoring.

Shingles will affect about a third of all Americans at some point in their lifetime. However, while many people have heard of the disease, fewer know much about it. Because shingles is so common and has the potential to produce painful complications, its important for everyone to understand the basics of this viral infection, including its close relationship with chickenpox.
Both diseases have the same origin, but there are important differences. Understanding these distinctions can help avoid confusion:
- Shingles and chickenpox are different infectious diseases, but both are caused by the same virus, called varicella-zoster .
- Chickenpox symptoms are the bodys first reaction to a varicella-zoster infection, presents as a severe skin rash, is more common in adolescence, and is highly contagious.
- Shingles symptoms are the bodys subsequent reaction to the same varicella-zoster infection that reactivates in the body, often many years later. It is more common in adults and presents as a painful rash as it strikes the nerves. You cant get shingles from someone who has shingles. However, you get chickenpox from someone who has shingles if you never had the chickenpox vaccine or chickenpox earlier in life.
How To Treat Shingles
Patients always want to know how to get rid of shingles. Shingles treatment is usually started once a person is diagnosed, ideally within 72 hours of the start of the rash. Antiviral medications for shingles, such as acyclovir, famciclovir or valacyclovir, are often prescribed for one week or longer, if needed.

What Are The Signs & Symptoms Of Shingles
Often the first shingles symptoms happen in the area where the rash will appear. A person may have tingling, itching, or pain in this area. When the rash shows up, the pain may be mild or severe.
The rash starts as groups of tiny pimples on one side of the body or the face. It’s often in the shape of a band or belt. The pimples change to pus-filled blisters that break open and scab over in about 710 days. The scabs usually heal and fall off about 24 weeks after the rash starts.
Some kids with shingles also may have a fever and a headache, and might feel tired and achy. Rarely, a child has the pain of shingles without the rash. More severe symptoms can happen, but usually in people over age 50.
Can You Have Shingles Without The Rash

Is This Shingles Without A Rash Worried
About 5 weeks ago I injured my back, don’t remember doing it but it felt like a pulled muscle. I thought nothing of it but then 2 weeks ago I got tender skin on patches of my back and around to my chest. I thought maybe it was a trapped nerve but the back pain is now just about gone. The skin pain is still there, also my right lymph node is enlarged and the skin around it is also tender. The skin on my back and around to my chest feels like it is tender too , like mild.sunburn or goosebumps, it is on my left side. I went to the doctor but he said there’s no rash then it’s not shingles. He sort of put it down to nerve pain but im not convinced. The last few days I feel a bit tired too. There is no rash whatsoever but the skin pain is very painful at times especially on the groin where the lymph node is.
